1) Bring your Live Scan request form 

(issued by the school, employer or who has requested your Live Scan.)

 2) Bring one of the following types of valid ID.

(California recognizes the following types of identification if issued by the State within the last 5 years. It must contain a photo, physical description, signature and serial number. California driver license or ID card. Driver license or ID card issued by another state. Mexican or Canadian driver license. US passport. Foreign passport stamped by US Immigration and Naturalization Service. US Military ID card.)
